My online scheduling is done using ClinicSense. You may need to create an account. Once you're signed up, you'll be able to schedule appointments, make purchases, and manage your account at your convenience.
Please note: My online form is only an appointment REQUEST. You will receive a second email or phone call to approve your request within 48 hours of submission. If your request cannot be accommodated, an alternate date/time will be offered. You may submit more than one request.
I make all efforts to respond to requests by the end of the business day or as soon as possible. If you make an appointment request and do not hear back from me within 48 hours please feel free to call/text/email me directly.
